Port Wine Birthmark

My stepmother has a very old faded blue tattoo and can not afford laser surgery and does not want a tattoo cover-up. She is ready to make it look like a kind of birthmark port wine or something, but I'm not sure that would be covered? (She is Tan BTW)
All tattoo artists ever do anything like this before?
